&lt;p&gt;I&#039;m t2 and 15 weeks pregnant, so we&#039;ve added Lantus to my regimen and will eventually add a fast-acting insulin as well.&lt;/p&gt;
&lt;p&gt;Your sheet is so nicely done that I can&#039;t think of anything I&#039;d change. I&#039;ll let you know how things play out as I use it. I&#039;m betting that a 5-day version, with the weekends on a separate sheet might be more beneficial -- weekend trends don&#039;t match workweek schedule in my case. It&#039;s just so neat having it all on one sheet, though!&lt;/p&gt;
&lt;p&gt;Torn, is what I am. And hungry. Off to find some lunch.&lt;/p&gt;

 <title>Planner Pad solution</title>

 <description>&lt;p&gt;I really like the Planner Pad format, too, especially after I quit using them for a couple years. I planned to buy a new set this fall but am put off by the &quot;special&quot; price. It&#039;s Sept and the &quot;special&quot; for Apr08-March09 verion is still more than 1/2 price for a full year. Used to be you could get some undated or end of year pages free or for a few bucks if you bought a complete new year.&lt;/p&gt;
&lt;p&gt;The Planner Pad style templates others created and graciously uploaded to DIY are nice but pages are too small for my good leather Personal Pages size binder. However, I did come up with a solution...and without spending hours of trial and error trying to create my own template.&lt;/p&gt;
&lt;p&gt;I found an old spiral bound Planner Pad from 2003 in my files, and luckily found a two page (one week) spread with only a few notes and used white out to delete them. &lt;/p&gt;
&lt;p&gt;Then I:&lt;br /&gt;
1. Scanned each side of the two pages into a separate graphic files.&lt;br /&gt;
2. Inserted them into Publisher to re-add a few blocked out lines.&lt;br /&gt;
3. Placed white boxes over any lingering smudges.&lt;br /&gt;
4. Placed a box over the old tiny monthly calendars in the bottom corner of the right hand page.&lt;br /&gt;
5. For each page of graphics, selected all and grouped the image into one image per page.&lt;br /&gt;
6. Choose Save Picture and resaved each jpeg image, one left page and one right.&lt;/p&gt;
&lt;p&gt;I inserted the two images into two different pages in Word, used narrow margins and right align for the righthand page, and left align for the lefthand one. Then printed both pages out on nice bright white paper. There&#039;s plenty of room for 3 or 7 ring hole punching. Or you can rescale the image to fit a Circa size. And, saved the Word file as docx, doc and template formats. :)&lt;/p&gt;
&lt;p&gt;I&#039;m pretty happy with the quality of the print out even though I need to hand-write the months, week spans dates, and daily dates. I&#039;m planning to have copies made on decent weight paper. Just need to trim paper a bit, punch and I&#039;m ready.&lt;/p&gt;
&lt;p&gt;I considered uploading my images but since I didn&#039;t create anything, just scanned in originals I figured that would be a copyright violation. But, I thought others here might like be interested in my DIY solution. :)&lt;/p&gt;

&lt;p&gt;A lot. &lt;/p&gt;
&lt;p&gt;The first thing I did was take out the big numbers and the holiday graphics. Then I changed the font to Blue Highway. Then I translated the whole thing to French and I took out all the names of the holidays because they were American holidays. Then I changed the margins a bit and printed all the pages on the left page. On the reverse I printed grid note pages for the right pages. That way, it&#039;s closer to a Modo e Modo moleskine weekly planner. I smurfed the hole thing and added it to my circa classic planner.&lt;/p&gt;
&lt;p&gt;And I love it.&lt;/p&gt;

 <title>Took me awhile to learn how</title>

 <description>&lt;p&gt;Took me awhile to learn how to &#039;zip&#039; and &#039;unzip&#039; *smile* but the pdf results in gobbly-gook symbols!  ugh.&lt;/p&gt;
&lt;p&gt;but yeah, me, I was able to view the documents on &#039;open office&#039; which I downloaded last night.&lt;/p&gt;
&lt;p&gt;trying so hard to learn.&lt;/p&gt;
&lt;p&gt;forms look great!  &lt;/p&gt;
&lt;p&gt;what I&#039;ve done is ask my grocery store manager to send me the store layout on excel, and then I made a spreadsheet front and back of everything we buy.  we highlight what we want to buy that week.&lt;/p&gt;
&lt;p&gt;my menu plan isn&#039;t a blank form either, I have a hard time planning from scratch each week.  I have a form with everything (almost) I know how to cook - and that&#039;s limited to one page) with all the main dishes separated by category.  famiy knows to pick one beef, one chicken, one salad etc.&lt;/p&gt;

&lt;p&gt;I didn&#039;t need anything fussy with appointment slots as this is for my own personal life nothing to do with business - and now I have it saved as my own template with custom bits it will be used for years and years to come.&lt;/p&gt;
&lt;p&gt;You&#039;ve saved me a fortune both in time and money (and given me happiness in not having to &#039;put up&#039; with a shop bought one that didn&#039;t really work for me) and for all that, I am eternally grateful!!&lt;/p&gt;
