Thanks for doing this; I had just sort of assumed I wouldn't be able to find anything like this. I am still interested in making a DIY planner someday, though, so a southpaw version of that would be nice.

For implementing GTD you might also try out this web-based application:

Gtdagenda.com

You can use it to manage your goals, projects and tasks, set next actions and contexts, use checklists, schedules and a calendar.
A mobile version is available too.

As with the last update, now Gtdagenda has full Someday/Maybe functionality, you can easily move your tasks and projects between "Active", "Someday/Maybe" and "Archive". This will clear your mind, and will boost your productivity.

Could you save the template (save as) in a previous format/version of OneNote?
In this way more people could use the template, where now only the OneNote2007 owners can.

I like that we can have business style and more feminine both represented on here. That design is lovely! I find I use my planner more when I enjoy looking at it.

"beautifying" doesn't have anything to do with being productive, but I find that if what I'm work with is visually pleasing to me, I'm more likely to stick to it.

Agree 100%. The look of a planner is as important as the philospophy that underlies it. I've written in other posts here that I find the standard D*I*Y Planner pages to be visually distracting; too much black, rules too thick and heavy. My preference is for rules to be (light) grey and thin, which is why I'm using the filoFax Time Managment pages as they are just about right though some of the elements could do with alteration.
