I cobbled this together from other ideas I've seen out there.

This has sections for the project overview, goals, etc.

The Steps/Action Items section has What, Who and When. There are notes on the back page, with a follow-up section which also includes What, Who When,

To make a "multi-page" project:
1. Print a double-sided Project Planning page.
2. Print as many double-sided Project Planning Notes Pages as necessary.
