Planning patient visits

I'm a hospice chaplain covering a large geographical area. Even though we use Electronic Medical Records it is inadequate for planning my schedule and informal recording of visits.

Does anyone in the group have experience with anything similar? I've tried several different tools right now I'm simply using pen and paper with ad hoc structure. This may be the best I can do but I thought I'd cast the net out here to see what could be caught.

It's hpda sized, which might be just what you need.

I know I saw a really good one recently, that had lines with room for dates, and topic, etc. But I don't remember if it was here, on Levenger, or somewhere else... I'm still looking. If I find it, I'll report back. :-)
