American Sign Language Vocabulary Journal

American Sign Language Interpreters, and students of the language, often keep a vocabulary journal to document new signs learned or unusual use/interpretations. I've created an "index-style", 3 cards per page, 2 pages per sheet template in .pdf format.

Apr - June 2010 Daily Calendar Pages - Letter size

Daily calendar pages for April, May and June 2010 with columns for "Priority To Do" and "Want to Do," as well as an "Appointments" column with times (my day starts at 4:45am so the first time row is 4am). There is a small area to write in Breakfast, Lunch and Dinner, and a "Notes for Today" area to put things like "Hubby out of town" or "Jane's Birthday."

I’ve added some more spiritually-oriented quotes as they have personal meaning for me. But if you find that the quotes are inappropriate or don’t fit your personal belief system, then definitely download the original .odg files and put quotes in that you find more appropriate or personally meaningful.

I'm sorry for the wonky margins. OpenOffice Draw (and the consequent pdf that I make from it) is pushing the entire page to the right by one-quarter inch, and I cannot figure out why. So I'm just manually adjusting for it.

Authorization for Disbursement envelope

This template is printed on letter paper (8.5" x 11"), cut and folded into an envelope that fits into a Classic binder (5.5" x 8.5").

The envelope has a form on the front for users to list expenses for reimbursement, disbursement requests, and check payee and address information. There is also a place for 2 authorization signatures on the front form. The envelope includes instructions for use on the back as well as a large place for notes.

Paper size:
Classic (5.5 x 8.5)
Thumbnail:
Auth_for_Disbursement_env.jpg
Usage advice:

This form is the template of an envelope with forms printed on it. Print to 8.5" x 11" paper (landscape), cut, fold, punch, and store in Clasic size binder (5.5 x 8.5).

I use this form in an organization where I am the treasurer. Members complete it to submit receipts for reimbursement. Members also use it to request a check for something that has a quote or purchase proposal.

As an envelope and form in one, it is really handy for someone like an organization's treasurer or finance committee. It keeps signatures and receipts/quotes all together in one neat place and package.

The template here is simply titled "Authorization for Disbursement". In my customized version, I put the organization's name above the title and the organization's logo to the left of the title. That kind of customization gives the form a more official feel when the form is eventually used for official financial documentation.

Basic Book Review Schedule

This is a basic book review template, that I created, to keep track of books that are being reviewed, for whom they are being reviewed for, and when they are due, to help keep track of what is needed for each month.

Right now this is a very basic, to the point. It's 8.5 x 5.5 and meant to be something small and easy to use. It can be printed out for all the months, or enough to bind together to have a running record of books reviewed/when.

Paper size:
Classic (5.5 x 8.5)
Thumbnail:
Basic Book Review.gif
Usage advice:

This can be printed double-sided to save paper, or printed individually to place in its respective month for a quick overview of that month.

This is to help keep track of books that are being reviewed and if, one finds that they have a lot of books to review for the same month, I would recommend, just simply print both sides. It's meant to just be enough of a quick glance over.

Basic Work Week: 2010

I work an 8-5 job, 5 days a week. Work related appointments / tasks / reminders are handled electronically in Outlook. However, I still like to have a basic overview of the working week down on paper. I originally started using chefz' Version of the Planner Pad: http://www.diyplanner.com/node/6597

That was still too detailed for me, so I have come up with my own Planner. I am sharing it in case anyone else is interested. I have attached the Excel file as well for those who want to edit it. The PDF covers the whole of 2010.

Bill Tracker

I have a hard time keeping track of which bills I've paid and which bills are outstanding. My AHS (Any horizontal surface) method of organization can be a bit chaotic at times, and while I've never had anything shut off (That one time with the internet doesn't count, right? It was their fault after all.) there have been times when it's been a bit dicey.
So, in order to combat this tendency, I made this template to help me keep track of who/what/when/where/and how much.
It's only got a left-hand space for holes because I don't generally use the back sides of pages. I know, bad precycler. I think it would be good to stick this on a pocket page too, to hold the bills!
I'm still figuring out the widget so I'll probably upload a classic size in addition to letter, and provide a second page with the hole space on the right later.

Paper size:
Letter
Usage advice:

Gather together your bills and make a page for each. You can put the average amount you pay each month in the notes if you like. That can make it easier to budget.
Fill in the name/address/phone number/account number fields right away.
When your bill arrives each month fill in the amount due and the date due right away.
When Pay All The Bills day arrives, fill out the amount paid and the date paid fields.
Payment method allows you to state if you paid by check, internet, or phone. Confirmation number is for the confirmation number you get when you pay by phone or 'net, or the check number you used.
The notes section is helpful for when you have to call customer service and you need a space for all those ticket numbers, (In a billing spat with AT&T I discovered how helpful keeping track of that sort of thing is.) or to note any extra fees associated with the bill. Late fees, debit card fees, pay by phone fees, etc.

Blank Checklist v3 - 2up Front and Back

Book At A Time Bible Reading Plan

A template designed to aid in your bible reading. It lists books of the bible by "average reading time" and provides a place for notes as you read though the bible.

Paper size:
Classic (5.5 x 8.5)
Usage advice:

The template will give you an overall view of how long it takes to read through each book of the bible so as you have time, you can look at the "average reading time" on the template to pick the books you'd like to read depending on your time available. I've used this on long flights to read an entire book of the bible as I've flown. It's quite helpful and only takes three classic size pages (double sided) to list all 66 books and leave space for notes. For a specialized reading plan that you can use in concert with this template, see Woodrow Kroll's book "Read your bible one book at a time".

Business Card Case - Origami Style

Create your own six-pocket case using one letter or A4 sized piece of paper. Holds up to 24 business cards or about 6 credit cards.

Paper size:
Other
Thumbnail:
Usage advice:

The attachment includes step-by-step instructions with photographs so you can fold your own business card case from a single piece of paper.

I used a piece of paper that had a printed pattern on one side and plain white on the other. However, both sides will show in the final product, so you may want to find paper that has color/pattern on both sides.

The folds are simple and there is no cutting. The result is a small paper rectangle with six pockets--four flap pockets and two full-depth pockets. The pockets hold everything reasonably securely, but if you're nervous about it you can be extra sure with a binder clip, rubber band, or some other extra device if you wish. This is not a 'jotter,' it's just a set of pockets.

You can stuff more than 4 business cards into each pocket, but if you do that in all the pockets it gets pretty tight. About 4 cards in each of the 6 pockets is still reasonably convenient and easy to put in/take out, etc.

If you have a small clip pen, you could attach the pen along the 'spine' of the case when you're done folding, if you wish.

Business Card Cases - Crayon Box Style

Two boxes for holding business cards or credit cards. One holds about 30 business cards, the other holds about 60 business cards. These boxes require a bit of glue to hold them together.

Paper size:
Letter
Usage advice:

These boxes are good for purse organization as well as mini-hipsters. They hold credit cards or business cards as well as business-card-size-hipsters. Use different colored boxes to differentiate different types of cards--medical, membership, punch cards...

Print them both on heavy cardstock--I used 110lb Card Stock, not the lighter weight Cover Stock. Do them both just in case you decide you want a bigger or smaller box once you've finished folding one.

Cut out the whole thing around the outside edges. There are four little spots to make additional slits, these are marked with arrows and instructions on the template pages.

Score the creases with the dull rounded edge of a butter knife. Use a ruler to guide the blade in a straight line. This will make the folding easier and cleaner later on. Trust me, you want to do this step.

Glue the long trapezoidal tab to the inside of the fat rectangular tab on the opposite end. This will form a tube. I used glue stick for this. Double-sided tape would probably work also.

Fold in the little side tabs on one end of the box and tuck in the rounded tab. Then put your business cards into the box and close the other end the same way.

If you want greater security than just tabs and slots, you can use binder clips on either end of the box to make double sure it stays closed. Ribbons and rubber bands will also work just fine.

Some options:
* You could maybe punch a small hole in one corner of the box to add a ring to it so you could attach it to a keychain. Not sure how this would work long term, but you could try it.
* You could add a pen loop on the side of the box by adding a two inch by one inch rectangle to the box template and forming it into a loop with glue.
* You could cut some small diagonal slits in the four corners on one side of the box to make a 'jotter' arrangement for a card on the outside of the box. Just don't get them too close to the edges. :)
* You could add a 'hang tag' to one short end of the box by adding a one inch wide tab along the edge of the fat rectangle, folding it in half (glue it down), and hole-punching it. This would probably work better than hole-punching through the body of the box.

Business Card Sources Page

Pages desigend to give you a place to literally stick business cards for places you shop. Requires Double Sided Tape.

Paper size:
Classic (5.5 x 8.5)
Thumbnail:
Usage advice:

This is a sources page for lazy people (like me) who aren't going to recopy data that is already in business card form.

If you print it out and apply double sided tape to the striped areas you will be all ready to add business cards to your planner's referance section. When you have a target card take off the other side of the double sided tape, stick and you are almost done.

The business cards attach sideways, but there are 2 lines either underneith or above to write a quick discription (eg Cakes, Web Guy) and a little circle to rate the service (the more colored in the better).

The Draw Tempale offer 3 other ratings squares (5 stars, single starfor numberical rating and a "happy face" without a mouth).

Business Expense/Mileage Logs

These forms are designed to work together. I used these forms (though not in DIYPlanner format) for a number of years, and can attest that they work. In fact, my accountant loved this format. :-)

I have to confess, however, that the basic format is not unique to me. In fact, this form is loosely based on a Dome book. However, I constantly worried about losing my Dome book. So, one year, when I couldn't find the Dome book in the store, I made my own forms that fit inside my DayTimer, and created a spreadsheet that would work with my own forms. I have now modified my forms yet again to take advantage of the Widget kit, and make them more general, and less specific to my needs, including adding extra categories, and leaving some blank lines for those who need even more categories.

I've enclosed instructions in the zip file.

Enjoy!

Calendario Tamaño Classic 2 Páginas Por Mes - Classic Two Pages per Month Calendar

Aguardo comentarios.

It´s pretty simple (that´s my goal) but very customizable, before or after printing. You can take the original file and translate or chage it the way you want it. I add a lined notes page if you want to print them in the back of calendars.
