CALORIE INTAKE AND EXERCISE RECORD

I needed a place to record my daily caloric intake with the kind of exercise done. I created this template to help me. It has Mon-Wed on the front and Thurs-Sat. on the back. I take Sundays off. I thought it might be of use to someone else. I do not claim that it is perfect, I am new at this. :)

Paper size:
Classic (5.5 x 8.5)
Usage advice:

Notes:
When printing each page, BE SURE to print only the first page, NOT the second page. This applies to both sheets. I could not delete the frames on the second page or the 2nd page sheet itself, so just ignore it. Refeed the printed out sheet number one through again and print sheet number two on it. REMEMBER TO ONLY PRINT SHEET ONE FROM PAGE ONE AND PAGE TWO.

The border is exactly the size of 5-1/2" x 8-1/2" so you may use it for a cutting guide. I also used a cutting corner punch for the corners as I prefer the corners curved. I picked up the punch at Walmart for three bucks.

I have found this useful for me and I hope it will be for you.

Pen and Ink Inventory

This is a form for the penaholic. The front is 1) an inventory of pens, by brand, name, type (fp, bp, etc) and point (fine, med, broad) and current ink color and 2) an inventory of inks by brand, name, form (bottle, cartridge) and comments. The back is lined for samples of inks and pens listed on the front.

Q&D 2 page per day calendar

I use the 2-page system with simple scheduling & task lists on one page and a diary page on the other for various things, such as daily meeting notes. I've found the stuff on the site very interesting - but one of my frustrations has been trying to get calendar pages printed correctly. The Dynamic Templates app is pretty good, but personally I think it needs some refinement - after trying to make it print usable pages, I gave up & created this very simple page setup for a generic, undated calendar & diary page using OpenOffice's Calc (spreadsheet application).

It's black & white, and pretty plain looking - I'm printing on a B&W laser, so I didn't want color. However, it's easy enough to customize this, as well as editing the fields for time of day (my shift starts at 7AM). Granted, it's not very flashy - I also worked a bit with the DIY kit, but found it cumbersome to work with - but it does what it's supposed to do.

Feel free to customize it any way you see fit, and please upload any cool variations for others.

Paper size:
Classic (5.5 x 8.5)
Thumbnail:
daily calendar page.png
Usage advice:

Note that this will print pages with the schedule on the right-hand page, while the diary is on the left. This works for me, but it's backwards from the Day Timer page I patterned this after.

[EDIT] If you want it the other way around, then I suggest adjusting the inner & outer columns. Notice that the image has the wider margins (1/2") on the outer edges of the page. If you make them smaller, and then make the center columns on either side of the cut line 1/2" wide, you should be good to go. The two 8-1/2" x 11" pages are created vertically (i.e. scroll down to see page 2) so your changes should reflect on both pages if you work with columns.

FYI, I made it 2 pages so I could easily do duplex printing.

Also, row height should be .25" all the way down.

A5 Filofax Day Planner

I had some preprinted pages like this one in the 1980's in my Personal size Filofax. Having converted to an A5 binder a couple of years ago, I decided to recreate the page for A5 usage.

Feel free to edit the page to meet your own requirements.

Paper size:

Usage advice:

I've found this useful to planning my day when I seem to have more tasks ahead of me than I think I can manage. Or I use it at the end of the day to plan the next day.

day plan

Index card sized planner pages for planning each day.

Paper size:
Index Card (3 x 5)

Usage advice:

Can be printed on card stock and cut. I print them to regular paper and paste each day into my pocket journal. Then I use as many following pages as I need to capture stuff for that day.

I keep a full to do list elsewhere, on this form I just pick out the "must do today or the world will end" items. On the timed page of the form I keep track of what I intend to do, what I actually do, and how long I do it. For work I have to keep track of what project I work on in 1/10th hour increments and there are faint lines at every 6 minutes.

Semi-Auto, Perpetual . One page per day, with quotes - Classic Size Only

This is a tweak of the awesome template uploaded by leecop. I made a couple of tweaks. First, it is now a Excel 2007 file. With the newer version, I was able to put all the colors, line styles, fonts, etc into a style sheet or theme. So, when I feel like changing all that, I just have to do it in one place.

Second, I took out the pictures and put in a quote box. This pulls quotes randomly from a sheet I added that has the motivational quotes I like. Add and remove quotes from the master list at will. A great many of these quotes came from the aphorism thread on this site.

Third, pulled it all into one sheet within the workbook, arranged to do 2 up duplex printing.

The one hitch with the quote feature is that it is truly random, so you can get duplicates.

Paper size:
Classic (5.5 x 8.5)
Usage advice:

follow leecops instructions in the file itself. But, you only have to print the one sheet.

The Perpetual Strip

While browsing around a local thrift store, I found a pen cup with a perpetual calendar along its side. I realized that this would be a great low-budget calendar solution for my desk, so I don't need to constantly hunt around to figure out when a certain day falls in the current month.

Paper size:
Letter
Thumbnail:
Perpetual Strip Thumbnail.png
Usage advice:

Print out the attached document, and cut along the solid lines. Pass the strip of numbers through the slots in the frame so the numbered days line up under their correct position in the current month. Sunday-started and Monday-started weeks are included, and you can easily tweak the spreadsheet files to customize for size, language, font, and general attractiveness.

I printed out mine on bright card stock for durability, and so it won't get lost on my desk. I also reinforced the two slits in the frame with some tape before cutting through them with a sharp knife.

Weekly Undated Calendar in Word with Checklist

This is in Monday - Sunday format. It's very striped-down & customizable and the spacing is small. I just added my own personalized checklist to the right side of each day. There's a blank box after Sunday with lines for notes.

Paper size:
Classic (5.5 x 8.5)
Usage advice:

I created this in Word after admiring a Lefty template by Alison Kleinschmidt. It's not as pretty as hers, but I wanted a personalized checklist on the right side of each day so I could track sleep, meds, lunch, working out, and connecting & preparing. I just found out that I'm bipolar, so I'm hoping this will help me track a few daily tasks & keep me focused.

After becoming frustrated with trying to learn a new program just to make a template, I decided to go with what I know. Word. I hope you enjoy it!

Weekly Action Sheet

A double-sided template designed to help you keep track of your main goals and to-do list for the week.

Shows Aims, Deadlines and Daily Goals on the face and a To-Do List on the rear.

Paper size:
A4
Thumbnail:

Usage advice:

I use an electronic device to manage my diary (linked to my networked office calendar) but find that this 'hard-copy' sheet that I can physically see and touch on my desk helps me to keep an eye on those pressing issues that don't appear in my diary.

I take the opportunity at the weekend to review my priorities for the week ahead and to transfer any unresolved items from my previous to-do list to the new sheet.

Tabbed Planner Dividers

This template came about because I needed some dividers for my planner that I'm setting up (yay!) and I'm not a big fan of the plastic stick-on tabs... not sure why, they just rub me the wrong way. SO, I decided to try to make myself some out of cardstock, and after much trial and error I thought, what the heck, maybe someone else can also benefit from all the work I put into this. I hope atleast someone, like me, is in need of tabbed dividers for a classic size planner, and that my templates can be some help!

The Open Office file is probably best because you can actually type in your own tab labels, but if you don't have OO, the pdf file will give you the basic print-and-cut outlines to get ya going.

Paper size:
Classic (5.5 x 8.5)
Thumbnail:
Tabbed Dividers Preview.jpg
Usage advice:

Instructions: Print on two separate sheets of thick paper (card stock). Cut along the solid lines, arrange, and hole punch to fit your classic size planner (5.5” x 8.5”).

(Note: the tabs will stick out 3/8” from the other pages in your planner, but the rest of the right edge will be slightly shorter than the other pages. This is normal. It has to be that way to be able to fit 2 tabs on one 8x11” page.)

Caution: I had to change my printer settings to “Borderless” to get these to fit right on the page. I advise running a test page with scratch paper before you try it on your nice cardstock!

Enjoy! Feel free to leave a comment with any questions or concerns.

Tabbed Planner Dividers

These are tabbed dividers (I've always called them "tabbies", is that odd?) that you can print and cut out yourself, 2 to a page, for classic size planners. I'm not a fan of the plastic stick-on things, for some reason I like plain old-fashioned card stock paper dividers, so I bothered to make this.

Paper size:
Classic (5.5 x 8.5)
Thumbnail:
Tabbed Dividers Preview.jpg
Usage advice:

The pdf file has the templates to print and cut out, and the odg file (which you need Open Office to open) has the blank templates AND the option of adding text to label your tabbies with.

Note: the hole punches in the preview picture are not actually part of the file, so if your planner has 7 holes or 2 holes or 86 holes, no problem!

Hope this is helpful to someone besides myself, and please let me know if anything needs fixing or whatever.

There are two Open Office files included. One is formatted for one per page and the other for two per page.
It is just a simple template, I hope you can use it. :)

Paper size:
Classic (5.5 x 8.5)
Usage advice:

If you can print classic size paper you can use the 1-up to print front and back. I usually just print one per page and place a classic size sheet in the upper left hand corner and trace around it and cut and punch.
