Interferon and ribavirin combination therapy side effects journal

This is a week on two pages journal for those on interferon and ribavirin combination therapy for the treatment of chronic viral hepatitis [Hepatitis C]

(Those looking for the Radio/chemotherapy side effects journal will find it here)

The 'Side Effect Journals' are powerful self-monitoring tools for use in conjunction with a personal diary.

Release notes: this is a potential Release Candidate [RC] for part of a range of 'free for non-commercial use' health care forms and is built around the 'free to use' DIYPlanner.com Widget Kit. However, I need feedback from users before settling on the final layout.

For example: is the form difficult to use and if so why? Does the form track everything you want it too and if not what would you like to see added?

International Radio Alphabet List (corrected)

I have a very long French name and I got tired of spelling it out only to have it mispelled anyway. So I made this card and now my name is not mispelled anymore. Ygor pointed out a booboo, so I fixed it.

Jan through Apr 2011 Daily Calendar Pages - Letter Size

Daily calendar pages for January through April 2011 with columns for "Priority To Do" and "Want to Do," as well as an "Appointments" column with times (my day starts at 4:45am so the first time row is 4am). There is a small area to write in Breakfast, Lunch and Dinner, and a "Notes for Today" area to put things like "Hubby out of town" or "Jane's Birthday."

I just put a link to my site because I was hitting the disk quota when I tried to upload the files.

Usage advice:

I'm sorry for the wonky margins. OpenOffice Draw (and the consequent pdf that I make from it) is pushing the entire page to the right by one-quarter inch, and I cannot figure out why. So I'm just manually adjusting for it.

January 2007 Daily 2-page forms

This package is a prototype of the compact (4.25"x6.75") 2-page-per day format of daily pages, designed in the theme of the DIY planner designed by Douglas Johnston. The second page is a right-handed daily journal page, to be
printed on the front of as many sheets as are required for the month. The subsequent pages are left-handed daily schedule and task pages dated respectively for each day of this month. They contain the date, weekday, and year; they also have mini-monthly calendars for the present month as well as the previous and following months. In addition, each day gives the phase of the moon and may note a special holiday or observance.

Paper size:
Other
Thumbnail:
Usage advice:

Print as many copies of page 2 as needed for an entire month's worth of daily journal entries. Then print pages 3-33 on the backs of those printed pages. You will need one extra page for the last day of the month; I recommend printing out 2-page-per-day monthly calendars as well (also available in the User Templates directory) and printing the Daily Journal for 31 January on the blank side of the left-handed page of February's monthly layout. (So you'd see a daily layout for 31 January identical to the rest of the days; when you flip the page, there's the two-page February monthly spread. This can also be done at the beginning of the January pages: on the back of the January monthly calendar, print the daily schedule for 1 January.)

Japanese writing paper ("genkoyoushi" - adapted from incompetech.com)

Japanese writing paper (genkoyoushi), adapted from the page generated at Kevin MacLeod's Free Online Graph Paper/Grid Paper PDFs page.

Paper size:
Classic (5.5 x 8.5)

Usage advice:

With Kevin MacLeod's permission, I've adapted this Japanese writing paper (genkoyoushi) to ensure there's enough room for hole punching.

The 1-up file includes front and back for duplex printing on 5.5"x8.5" paper. Printing the 2-up file with a duplex printer requires that the printer be set to long-edge duplexing, as the first page is upside down with respect to the second.

Java Weekly Planner - Standard

Upon looking for templates to use for GTD, I was a bit disappointed with finding that there were some templates that were extremely useful, but lacked beautification!

Now I know, you're supposed to be productive and "beautifying" doesn't have anything to do with being productive, but I find that if what I'm work with is visually pleasing to me, I'm more likely to stick to it.

Hence the creation of the Java Weekly Planner. This is a weekly, undated planner with a separate space for each day of the week as well as an area for notes directly across from it.

I have created a whole series of these - they are available at my blog:
http://freshfocus.info/blog/my-gtd-templates

There are many GTD tips and articles there too.

This is my first template, and critiques are welcomed and encouraged. Thanks for looking!

Knitting Project Manager

A basic form for knitters to use in organizing projects. There are sections for keeping track of patterns, yarn, and needles as well as room for sketches, notes, and charting.

Paper size:
Classic (5.5 x 8.5)
Thumbnail:
Usage advice:

I'm not a graphic designer, so this isn't perfect, but the basic idea is that you can have your project info all on one page for quick reference. I like to have records of the projects I've done, and this form can be used as a record of past projects as well as an idea sheet for future knitting. The back of the form is knitter's graph paper for charting color or stitch designs.

Left-Handed Weekly Calendar

A "Weekly Planning" template made for us lefties who use classic ring-bound planners.

Paper size:
Classic (5.5 x 8.5)
Thumbnail:
Usage advice:

My boyfriend bought me a beautiful Franklin Covey leather-bound planner for my birthday. I was thrilled; finally I had something beautiful to put my D*I*Y Planner pages in. Problem is, I'm left-handed, so writing on the right-side pages is a real pain.

This template is made so that the work days of the week (Monday-Friday) are on the left. It's currently not in the D*I*Y Planner style, because I'm redesigning more than just my calendar anyway. If there are enough lefties who want this in a D*I*Y Planner style, though, I'd be happy to make one.

I also included a checklist for the Weekly Review, and some reminders for every day. There are even four different snippets I took from GTD to remind/inspire me throughout the month. The snippets are different for each week.

It's in a 2-up format for the Classic-size planners, and if you print them double-sided, the holes to be punched will always be in the center of the 2-up page (so if you want to fold them in half and then punch them, instead of cutting, you can).

Let's Do Lunch!

Coordinate lunch time with this handy template for offices and laboratories.

Paper size:
Letter
Usage advice:

Lunch time is one of the best times to chat about interesting ideas and get to know your colleagues. Interdisciplinary research is born when people rub elbows, and innovative ideas can emerge from lunch-time conversations and scribbled-on table napkins. Get together with your coworkers today!

Print out and laminate this lunch coordinator, then stick it on your refrigerator or whiteboard together with a dry-erase marker. Suggest some times and encourage people to write in their names and the time they plan to head out, heat up their lunch, or call for takeout.

Levenger Multitask Organizer - Junior

My recreation of the new Levenger Multitask Organizer - I wanted to see if I liked the format before paying for the official versions. Refills are in my approximation of Junior size (it's really just two halves of a Letter sized document, but that was close enough for me). Word does add an extra blank page that I can't seem to get rid of - so only print page 1.

Levenger Multitask Organizer - Letter

I really liked the new Levenger Multitask Organizer setup, but wanted to see how it would work in Letter size (which is what I use at work). This is my attempt to recreate the look and feel of Levenger's form in MS Word.

Paper size:
Letter
Thumbnail:
LevMultOrg-LtrShort.jpg
Usage advice:

Uh - print out and punch? :-) It was saved as MS Word 97-2003 compatible and designed to take up two pages (meant to be open facing each other). I also created a version with all seven days on one page, but the columns were very narrow and it just didn't feel right. If anyone wants to see it I can post it as well.

Listy Planner (Classic)

For those who balance school, work, home/family and personal matters -- and love to do so by making lists -- this template combines list-making with a weekly planner page. This is my first attempt at a template, so suggestions are welcome.

Paper size:

Usage advice:

The page is 2-up and when you print/cut/punch, your holes will go through some of the lines on the template. I decided that I wanted more room to write overall, so sacrificed a little bit of the layout for that.
